NOTE: This form should be used only by individuals who have been contacted because we believe we may have your dog ALREADY in our care, or if you believe that a dog we FOUND could be your dog.

I am the owner of the pet listed below, and I have provided proof of ownership in one of the forms listed below. I wish to claim my pet and will take additional steps to assure s/he remains safe and does not become lost in the future.

Following is the proof of ownership you need to provide before scheduling a time to pick up your pet - many people claim to be the owners of lost pets, so we require verification:

ALL PETS: Proof of ownership required: A current Maricopa County license OR veterinary records (within the last 18 months) OR shelter adoption form OR implanted microchip reflecting you as the owner.

If none of these forms of legal proof of ownership are available, HARTT may require the owner to apply as an adopter - our normal adoption process will apply.

WHEN PICKING UP:
ALL DOGS: A properly sized collar, an ID tag with your name and number, and a leash. We additionally prefer that you bring a properly-sized crate for your dog to ride in for the trip home.
ALL CATS: A properly sized cat "breakaway" collar, an ID tag with your name and number, and a secure cat carrier.

RETURN TO OWNER FEES:
1. Owners are responsible for a $50.00 intake fee - this fee covers a small portion of the costs incurred by our nonprofit organization in searching for, locating and humanely trapping your pet, conducting an intake exam, administering first aid if required for minor wounds, cleaning ears and trimming nails, and providing topical parasite treatment if indicated)
2. $35.00 per night for discounted rescue boarding
3. The cost of all veterinary care (including treatment for illness or injury, spay/neuter surgery, vaccines, etc.) Please note that under Arizona law, all animals must be spayed or neutered before leaving a shelter; you will be responsible for the related veterinary costs if your animal is not already sterilized.
4. The cost of any required bathing, grooming, or dematting.

All animals must be reclaimed Monday-Friday between the hours of 8:00 am and 4:00 pm; our shelter does not maintain public hours, therefore you must have a scheduled appointment time 24 hours in advance so that we can arrange for someone to meet you. If you feel that you are no longer able to keep your pet, you may choose to surrender your pet to the shelter and we will place the animal up for adoption.

If your pet is microchipped, please write the microchip number in this field. If your pet was NOT microchipped, would you like us to microchip your pet for future security? The fee is $20.00 per pet. If your pet has a microchip but you never registered it to reflect your own information, you must do this before arranging to pick up your pet. Ask your HARTT representative how to contact your pet's microchip company to initiate a registration or transfer.*

I understand that HARTT is a small organization with limited space for holding stray animals with no ID. When we accept a stray animal we look for a tag, scan for a microchip and utilize other online sites and maps to attempt to find an owner who may be looking for the pet. However, if we are unable to find any information on the owner, due to space limitations we may elect to transfer the animal to another animal welfare organization or adopt the animal out after a period of 3 days. We do our best to hold the animal as long as possible, but given the number of stray animals in need, long stays may not be possible. (please initial):*

Because HARTT's mission is to secure lost pets and very difficult-to-catch strays, I understand that this pet is an extreme flight risk. Once an animal has become lost and is recovered, their chances of escaping again increases exponentially. I understand that HARTT may require a home inspection (inside and out) to verify that my property is secure so that this animal will remain safe in the future. (please initial)*
